BiliCalc App Description & Overview

What is bilicalc app? Designed by a pediatrician, BiliCalc uses the American Academy of Pediatrics 2004 "Management of Hyperbilirubinemia in Infants Greater than 35 Weeks of Age" to calculate the threshold for starting phototherapy based on the patient's age, bilirubin level, and neurotoxicity risk. Values are now displayed on the phototherapy nomogram. In addition, it will also tell you the patient's risk zone using the Bhutani Nomogram.

Allows direct age input and includes the option to calculate age from birth and lab sample times. The app has a direct link to source journal articles, nomograms, and a list of the neurotoxicity risk factors. Supports US and SI units (mg/dL and µmol/L).

App Feature Summary
-Calculates phototherapy threshold for newborns 35 weeks and older
-Displays values on phototherapy nomogram
-Suggests time frame for followup bilirubin levels based on patient risk zone
-Direct links to reference journal articles

Frequently Asked Questions
**********************************
1. I think the graph is backwards. Why is low risk on top and high risk on bottom?

The graph is directly from the 2004 AAP Hyperbilirubinemia Guidelines. The degree of "risk" refers to the infant's risk for developing neurotoxicity. This is different than the Bhutani Nomogram in which "risk" refers to the risk for development of severe hyperbilirubinemia. The threshold for treatment (when a data point lies above one of the lines on the graph) of the infant is different based on the predisposing factors (divided into the three risk categories). Babies who are "low neurotoxicity risk," can tolerate higher levels of bilirubin than a baby with medium or high risk. Therefore, a high risk baby will be treated at lower bilirubin levels than a low risk baby. The graph is correct in having low risk on top since a higher value is needed to pass the threshold for treatment (phototherapy). Please see the graph here for verification as well: http://bit.ly/AppPWU. Please email if you have further questions.

BiliCalc Comments & Reviews 2026

Outdated. Not updated to 2022 guidelines.

Wish the app would update based on most recent bili guidelines. Or at least send a notice out that they’re working on it

Really functional app. Will definitely be using this from now on.

Needs update. Excellent app just needs an update to reflect the new AAP guidelines.

Thanks for making this!. Easy to use. Helpful. A must-have for anyone in Pediatrics.

FREE and Perfect!. This is all you need. Easy to use and has a link to source material.

Nice app. Pretty cool app. I like it better than bilitool. I hope it stays free.

Fantastic!!. Have spent countless hours looking for just such an app. Finally gave up on finding a free, useful app to use and then saw this newly released app today. Thank you so much! Will be using this all the time!

Inaccurate. Intermittently inaccurate plotting of bili values on graphs. Dangerous if you don’t realize it b/c app plots bili at lower than value entered.

Wonderful!. Keeps time in newborn nursery and clinic at a minimum. So easy to use!

Ads? Come-on! Not free anymore.. I don't mind ads when apps are free BUT this one is not! Don't force me to look at ads if you're taking my money. Does well what takes you at least 60 seconds to do on paper and you don't have to carry the info around. I was hoping for the Bili level to be plotted against the nomogram. Oh well.

Needs updated to 2022 guidelines. Currently outdated. Loved the app when it was up to date.

Not up to date guidelines. It’s a great app and easy to use but needs to be updated to the 2022 guidelines.

Simple and Helpful. Easy to use. Very simple design gets you the info you need fast.

Pretty good- useful in Peds. I find this app to be accurate and useful in pediatric practice. I have 2 suggestions: 1) Plot the bilirubin level on the bilirubin nomogram. (If you could figure out a way to plot more than 1 point, that would be awesome) 2) When you select the option to enter the birth date/time and then the collection date/time, it would be nice if the app showed how many hours that represents. (in single hour increments). I like to be able to report the Bili level and the baby's age in hours to my colleagues, & this feature would save me a lot of time!

A handy tool. Direct links to sources is appreciated.

BiliCalc great tool for family physicians and pediatricians!. Very simple FREE app for determining management of hyperbilirubinemia using current guidelines.

Dr GK. Very user friendly, I especially like that you can swipe and see the graphical representation as well. I'll definitely use this as a wards reference.

Very Useful. Bilicalc is easy to use and very useful for any general pediatrician.

Not updated. Needs to update to 2022 guidelines

Please update to new AAP guidelines!!. Love love love this app but please update!

Very nice app. Easy to use

No longer accurately plotting risk. Using iOS 11. Do not use this app, it is no longer accurate.

Great!!. I used it every day on my nursery rotation. Thank you!

Details need work. Graph is backwards, higher bili level means lower risk? Instability is misspelled, other than that I hope the data is input correctly. Lack of details decrease confidence.

Great app, but.... Very useful app for docs and nurses; tells you what risk category the jaundiced baby in your nursery is. But would really be improved by showing how many hours since birth the lab time represents and by graphing it on the appropriate nomogram.

Great help. How is this free? Very helpful and easy to use. Great time saver.

Used to be a 5star app. The app still uses the old 2004 AAP guidelines and has not been updated to the 2022 guidelines. It appears to be effectively abandoned by the owner. There are other alternatives on the App Store. Is the app still functional? Yes… but it is no longer pertinent

Bili Tool. Not yet updated with the current guideline???

Updated guidelines. Can the app be updated to be based on the new 2022 guidelines?

Awesomeness. Well done! Great/useful app!!

DO NOT USE! Graphs wrong when on phone call.. If you use this app while you are talking on the phone on iPhone 5, it places the crosshair on the wrong spot on the graph making to bili look lower risk than it really is. Even after you hang up and enter other Billy Rubin levels, the error in placement process unless you close out the app from your memory. Most of the time that I need to look up nomogram, it is on the phone when I am on call, when a nurse or parent calls me with a bili. Therefore this app is useless to me, and can lead to bad decisions. The developer of this app has been unable to fix this in the last six months.

Simple and Very Useful!. Great new app! Love the layout. Simple to use. Links to the resources a bonus... And FREE! A must for medical students, residents, nurses, and practicing physicians.

Best medical iphone app ever!!!!. The app is super useful and free!!!!! Simple design but does all you need. I recommend this app for everyone- doctors, nurses and smart moms.

Great idea!. I am so glad there is finally an app for this! This is a must have for all pediatricians!

Super useful app. Very easy to use and will really come in handy. Thank you for this!!

Useful Application. Great and Simple to Use. Thank for keeping it free.

Free app. Love this app. Someone has to pay for the creation and maintenance of an app. I appreciate the efforts of this pediatrician to improve care.

Excellent app, update due. This excellent resource needs to be updated to reflect the recent guidelines for managing hyperbilirubinemia in infants 35 weeks gestation and older

Updated guidelines 2022. I recently bought the app not realizing that it does not have the updated 2022 guide limes , can the app be updated ?

FINALLY!!. An easy-to-use app for checking bilirubins. Perfect for housestaff and attendings alike. I bet med students could get some use out of the reference links too.

Amazingly helpful!. This app made life SO much easier while rotating in the newborn nursery. It's extremely easy to use AND its free...even better!

Perfect app. Does just what it advertises. Perfect for any pediatrician or anyone who has a need to make this calculation quickly. Free as well. Can't beat that.

Great app. Incredibly easy to use with a great interface. Graphs correct and consistent with AAP recommendations. Essential app for newborn medicine!

Essential Pediatric App. This app is super useful for the newborn nursery. I use it daily. I always have my phone with me but now I don't need to remember to carry around a graph. I think the previous reviewer is confused by the graph. The low, medium, and high risk lines on the graph refer to the "neurotoxicity" risk. Thus a "high neurotoxicity" risk baby will need light therapy at a lower bilirubin level than a "low neurotoxicity" risk baby. This is exactly how the Bhutani phototherapy nomogram looks like.

MUST HAVE for any Pediatrician, Pediatric Resident or Med Student. Incredibly useful; simple and easy to use, this app pretty much does it all with regards to calculations for Bilirubin levels. The most helpful element is the Nomogram with live plotting; in the past I would have to use the paper nomograms, this app does all the hard work for me. HIGHLY RECOMMENDED for anyone who spend times with infants in the nursery, NICU, or pediatrician for early newborn followup.

Love the app but.... Would love it even more if it included nomograms for *transcutaneous* measurements as well!

Needs 2022 update. Great app back in the day. Can’t use any Ire as it is based on 2004 guidelines, not the updated guidelines.

Discrepancy. I just used the calc and got a LR. Cross referenced it with BiliTool and they showed HIR. That can't be good.

Out of date. This was a great tool, but unfortunately now completely out of date. Would be great if updated but appears the creator hasn’t changed it in 9 years

Md. l'idée est bonne mais erreur sur le graphique. Je retourne à la version papier...

Loved by Canadian midwife. Easy to use and converts to metric and imperial units.

nifty. great app!! super handy for on the wards, and love the new graph option! :)

Best $ spent. Best $2 I've spent in a long time. Great job.

Inaccuracies. In comparing this app to BiliTool.org I noticed some differences that are minute but still different. Specifically if you put a bili level of 15.5 at 109 hours you get LIR. Bilitool.org says this is HIR. I know the sources are different but it's hard to trust an app made by one person in the medical field especially when peer review is difficult to perform.

Great app for pediatricians/future pediatricians. Well-designed, easy to use, and evidence-based. You can input values by patient age or by birth time/lab time to save you some math when your brain is fried on night float. I don't have the neurotoxicity risk factors memorized yet so it's nice that they're easy to get to. When compared to BiliTool.org some of the recommendations may differ when the patient is right on the risk stratification line, but as attendings like to say, there's no substitute for training and clinical judgment.

Great app!. Love this app. Thank you Dr. Beniflah!
